If you have a complaint concerning the care you received or in regards to the operation of our clinic, you may contact our Management Team by email at info@humbervalleyimaging.com, fill in the patient survey below or provide your details to one of our staff members for our management team.  

We take all of your feedback with care and all complaints written or verbal will be investigated.   We are committed to provide a response within 10 business days. 

If for some reason you are not happy with the resolution your complaint, or if you feel we have not dealt with the complaint in a timely manner, it is your right to bring your complaint to the Patient Ombudsman under the Excellent Care for All Act, 2010.
